What Are Custom Orthotics and How Are They Different from Insoles?

The word "orthotics" covers a wide range of products, from pharmacy insoles to clinically prescribed devices made from a cast of your foot. Understanding what separates them matters when you are deciding what your problem actually needs.

Custom Insoles - General Comfort Support

Our custom insoles are pre-fabricated and ready to be moulded to your feet straight out the box. They're a great option for individuals seeking general foot support on a budget or for those who do not require specific support for foot or biomechanical issues.

Bespoke Orthotics - Clinically Crafted for Your Biomechanics

We have been handcrafting Bespoke Orthotics for over 35 years, designing each device from the ground up using a range of high-quality thermo-mouldable materials. This specialist in-house expertise, allows our team complete control over every stage of the process, ensuring each orthotic is precisely tailored to each individual’s unique biomechanics and clinical needs. Our custom orthotics play a vital role in both performance and rehabilitation, forming the foundation of treatment for a wide range of conditions, including overpronation, flat feet, Plantar Fasciitis, Achilles Tendinopathy, shin splints, patellofemoral pain, and Morton’s neuroma.

3D Printed Orthotics - Advanced Support for Complex Needs

Our 3D printed Orthotics are state of the art devices created from semi-weightbearing foot scans and produced by our external lab. Made from durable powdered nylon, CPP+ and Copolymer they offer exceptional support. Whilst their function and effectiveness are the same as our Bespoke range these are an excellent choice when we need to support higher loads or meet complex needs.

What Conditions Do Custom Orthotics Treat?

Orthotics work by changing how load distributes across the foot and travels through the lower limb. These are the four condition clusters where they make the clearest clinical difference.

Plantar Fasciitis

Plantar fasciitis produces sharp heel and arch pain driven by repeated strain on the plantar fascia. A custom orthotic offloads the inflamed tissue and corrects the mechanical pattern keeping the injury active - addressing the source rather than suppressing the symptom.

Overpronation

Overpronation causes the foot to roll inward excessively with each step, placing cumulative strain on the ankle, knee, and hip over time.  Orthotics manage this by providing medial support that guides the foot into a more neutral position throughout the gait cycle.

Achilles Tendinopathy and Shin Splints

Both conditions are frequently driven by abnormal load patterns rather than tissue weakness alone. Orthotics reduce the mechanical stress on the Achilles tendon and shin during activity, giving the tissue the conditions it needs to recover while you stay mobile.

Knee, Hip, and Back Pain

Poor foot mechanics alter the entire kinetic chain. Sometimes, where pain in the knee, hip, or lower back has a biomechanical origin, orthotics can shift load distribution in ways that relieve pressure at those joints - without targeting them directly.

Our Process - From Assessment to Same-Day Collection

Most clinics separate the assessment, manufacturing, and fitting across multiple visits. We do not. Here is how it works.

  1. Biomechanical Assessment - Your podiatrist examines your feet, posture, and movement patterns in detail. Pressure mapping and gait analysis inform exactly what the orthotic needs to correct and where load is currently concentrated.
  2. Casting and Fabrication - Depending on the orthotic type, your foot is either moulded or scanned digitally. The device is then crafted or printed with materials, thickness, and posting selected specifically for your presentation. From assessment to finished device takes around 90 minutes for our Bespoke range
  3. Fitting and Adjustment - Your podiatrist fits the orthotic, checks alignment, and makes fine adjustments before you leave. A follow-up is scheduled to confirm the device is performing as intended.

Who Benefits Most from Custom Orthotics?

Orthotics are not only reserved for athletes or severe presentations. These groups see the most consistent results.

  • Runners and cyclists dealing with repetitive strain injuries or biomechanical inefficiencies that affect performance
  • Office workers and professionals on their feet for long hours who develop heel, arch, or knee pain over time
  • Older adults experiencing joint loading changes, reduced plantar fat pad thickness, or post-surgical gait shifts
  • Post-injury patients returning to activity who need mechanical support during the recovery phase
  • Anyone with a structural foot issue - flat feet, high arches, or leg length discrepancy that affects daily comfort

Stop managing foot pain around your schedule. Book a custom orthotic assessment in London at our Great Portland Street clinic and leave with a device built specifically for how you move.
